Out of Home Youth Supports
Nurturing social and emotional wellbeing through care
Phoenix works alongside community services organisation to provide support to vulnerable young people living in home-based care, foster and kinship care, lead tenant arrangements and independent living. We aim to create meaningful engagement and relationships between workers and young people, to help ensure their safety, wellbeing and
development. We provide the highest quality of care to these young people, and help to support the success of their placements.
Our services include:
- In-home supports
- Transports to/from school, appointments and other activities
- Community engagement
- Supervision of family access
- 24/7 respite supports (excl. accommodation)
Our workers are fully qualified with a minimum Cert IV in Child, Youth and Families or relevant field, and a minimum of 6 months experience working with vulnerable people.
